Pricing includes:
- Pre-operative consultation and simple imaging;
- ASC facility fees, including OR and supplies;
- Professional fees, including surgeon, assistant surgeon, and anesthesiologist fees;
- Three PT post-operative visits at an IBJI location (evaluation and two follow-up visits for physical therapy and rehabilitation);
- First post-operative visit and simple imaging.
Pricing does NOT include:
- DME items;
- Advanced imaging and labs;
- Prescription drugs obtained on an outpatient basis (e.g., obtained at a pharmacy post-discharge);
- Services that are not medically necessary;
- Physical therapy and rehabilitation beyond the three included post-operative follow-up visits;
- Follow-up care required after completion of the surgery and discharge from the facility (unless otherwise specified).
